Not strictly true - I have just replaced my UK licence with a spanish one and my licence was due to expire the following month and I did not have to have a medical. I am 61 and my understanding is that you only need a medical if you are 70 or over. We received our new spanish licences just last week the whole process took just under two months which I believe is pretty good from some of the horror stories I have read in the papers. We did not use an intermediary as I do speak Spanish (which helps!).
Not sure why the intermediary mentioned above was taking the paperwork to Cartagena - I understood our Trafico Office to be that in Alicante.

The "within a year" may have applied to if you're 69 plus as that is actually when the license runs out, not the renewal date?. Don't know.
You can use any trafico to change your license. Cartagena will process multiple applications whilst Alicante would only do one per person so may be the reason. In our case, Nick did mine and his wife did my wife's at Alicante.
From making first appointment to receiving new licenses was about 2 months. I know, some horror stories say they can take up to a year but we had the same experience as you.
