Many thanks Marcliff, looks like he cant apply for an S1 until we actually move to Spain. We have completed 2 tax returns as we have to submit quarterly rental income returns so we are on the Spanish tax system. Does that make any difference?

Income tax returns, not rental tax returns. The rental tax won't show your worldwide income so it can't recognise your annual income. Prescriptions are worked out on your total income. Earn under €18,000 then you get free prescriptions (in the Valencia Region) if you are a resident and over retirement age, over 18k and up to 100,000k then you pay 10% with a maximum cap. Earn over 100k then you pay 10% but with a cap of €61.75 a month.
However, if you are over 65 then you will pay 10% of the cost if you are not a resident and, more importantly, there is no cap on the payments. If you are a resident you pay a maximum of about €8.50 a month but non-resident over 65 will not have a cap and it will be charged at 10% of the full price.
Example
Resident over 65 earning over £18,000 a year and having 5 prescription each costing 100 euro will pay a maximum of €8.50 a month.
Non-resident over 65 (no income information) will pay €50 a month for the prescriptions.

Under 65 with a annual income tax return of less than 18k pays 40% of the cost of the prescription , 18k to 100k will pay 50% of the cost and earning over 100k pays 60% of the cost with no cap.
If you are spending several months in Spain, try and get enough from your GP to cover. They are officially limited to giving you 2 months worth but some will allow 3 months.

I think all your questions will be answered on the link below:

Thank you Marcliff, I understand the situation clearly now. Yes we are bringing 3 months supply with us which is sufficient as we are in the UK for Christmas so can re-stock. It was more of case of if we fell ill while in Spain, we arent used to not being registered with a doctor. This is our first whole winter in Spain so I guess we will just have to play it by ear.
