what do they mean by a reciprocal basis , are we covered or not that is the question .

In other words, if UK treats the Spanish in UK for free then the Spanish will treat the UK citizens in Spain for free.

As I understand it, the S1 & EHIC schemes are 'charge back' schemes, so in the case of the S1 for example, the UK pays Spain (approx £2400 per person p.a.) to provide UK pensioners with health care.

I don't get this 'only if UK reciprocates' business.

In UK NHS care is based on legal residency and nothing else and that applies to all be they Spanish, from other EU countries, or elsewhere in the world, without discrimination.

To reciprocate for UK paying for health care for citizens living in Spain then would mean UK singling out and actively discriminating against resident Spanish citizens by denying them the free access to health care the rest of the population enjoy.

I can only think that the Spanish government don't understand how health care works in UK because if they did then they'd realise that regardless of Brexit their citizens living there have an defacto automatic right to it because of that one simple fact.

UK is instigating a "settled status" category. This is the same as permanent residency in other EU countries. If UK says only those of settled status will receive free healthcare then that is what is meant by doing the same (reciprocal) and only permanent residents in Spain will get free healthcare.
Then there's the current EHIC where all EU citizens (not from everywhere in the world) gets free treatment whether in UK or EU. If UK stops that then Spain will stop it.
That is what the reciprocal bit means.
You're just picking the legal residency bit but it applies to many others.
You may also have missed the bit about the new (well, re-instigated) universal free healthcare in Spain for residents which is the same as the UK does so I should think Spain does understand how it works.
As said, have a glass of vino and wait for a few weeks to see what happens.
Maybe then it'll be time for this:
