We use "Hits" which uses the Orange or Moviestar Networks (if one of them has a poor signal it tries the other network)
With "Hits" you have to top-up every 3 months whether you are in credit or not, this has never been a problem for us as we have activated automatic top-up.
We like the "Hits" system as all the family and friends are on it and there is no cost calling another "Hits" customer.

We use Hits and pay a one off small fee once a year to keep the phone unlocked, all your credit is protected but more importantly your phone is always available for use.
You can also set up an automatic top up, we put on 10€ automatically when the balance falls below 4€. Takes all the effort out of having a cheap PAYG phone. Also calls between Hits users are free.

I use lebara too and they're fine.
Works in the UK too.
Because I've heard of the "time rules" I just make sure i top it up every 3 months so as not to loose the number.
Only thing you don't get if you're outside spain is the bonus minutes which have to be used within 30 days

Can I please advise anyone about to use Hits Mobile to think again. They have broken their own terms and conditions and attempted to overcharge me admin fees (back fees they seem to call them) on numerous occasions over the past month or so.
Their customer contact service is appalling and they do not offer an ounce of understanding when dealing with their customers. I made a very small error whilst attempting to top up my mobile sim by 10€ online and when it was brought to my attention I immediately tried to rectify it with them. They are currently chasing me for 39.90€ and will not budge even though they have verbally agreed with me that I had made a small error.
They are not for budging and neither am I - My sim is now cut in half and been disposed of.
Use at your own risk.
