I don't think you will be able to - whilst not 100% sure. We do have both Now TV Spain & UK on test boxes previously and the UK Now TV box would not work in Spain - so based on that and we have Now TV Spain / Now TV UK (both Sky and more different than you think - Now TV Spain is poor, few channels little content) - I can't see them allowing the UK one to work in Spain given the service is in both countries and they don't want the crossover - or usually. Based on previous, I would expect you do need a VPN - though as have not tried recently, cannot say with 100% certainty.

Perhaps best to ask a technical or Sky forum. Fact is you can't get UK Sky officially in Spain (from Sky UK), so it would be a reasonable assumption the same would apply to now TV -as is Sky content, meaning it is likely to be geographically blocked on whatever you use Roku box, smart TV, app or otherwise - without a VPN.

From NowTV website
As well as watching all over the UK with no restrictions, you can take your NOW TV Passes with you to many European countries too, for up to 30 days at a time.
To enjoy roaming with NOW TV outside the UK, you must:
Have a UK payment card registered on your account
Have watched NOW TV on your account in the UK within the last 30 days
Be in one of the EU countries listed below – you can't watch NOW TV in any non-EU countries.

It's much the same as Sky UK - you need a Sky UK active account first - in the UK< before you can sneak your Sky box off to Spain :D It is not a permanent solution - clearly the 30 day scenario is to block anything other than holidaymakers, as you'll need UK address, bank account etc, but if you are a holidaymaker or only need for 30 days or less at a time & can do the UK address/bank account - then may prove worthwhile.

Same rules will apply to this as to any other box and app - as per Babu's post. You can't access the UK website to purchase in Spain, without seeing the 'not in your location' message pop up either. Just tried. Without VPN of course.

Now TV vs Sky TV: content
With access to ten live TV channels, the Now TV Entertainment package looks like a steal, however, subscribers don't get unfettered access to all programmes due to rights and licensing issues. This results in slating – a practice wherein you see only an apology slate instead of a programme. The general rule of thumb seems to be that if it's a popular US show you actually want to watch, it'll be slated.

10 channels with a load of content missing -bargain?

Conversely On Demand offers a goodly selection of Sky's homemade shows alongside some strong imports. It should be noted that episodes are only available for a few of weeks, which makes the selection look a little random. There's a limited selection of box sets, however, these change routinely and may only be available for a few weeks before vanishing.

Now TV Movies is a churning service, albeit one with a large repository of movies available – Now TV cites 800 currently on demand. You also get a curated selection of new-ish titles, up to 16 a month. These arrive in line with its satellite-delivered sibling.

https://www.youtube.com/watch?v=rQvG-EtpCfA VPN on Apple TV. Adding it all up, I suspect there are both more economical and better content options available. Though ultimately will depend exactly on what type of content you require.
