I am guessing you are relocating from U.K. as you haven’t already got a SIP card.
You need to meet certain criteria to register on the Spanish Healthcare System You need to be in receipt of certain benefits to obtain a S1 from the U.K. State pension is one of them. You will also need to be registered on the padron.
Once you’ve obtained your TIE you can register at the local INSS office

bobby19.
Are you a UK citizen? Have you been living in Spain beforehand, that is before 1st Jan 2021?
Are you going to be living here on a permanent basis and becoming resident?

Lots of different things affect how you get health care. You can use a GHIC (the new healthcard issued in UK for temporary and necessary treatment).

If from outside the UK, including Ireland, you can get an S1 from your national health authority if you are over retirement age in your own country which will get you full access to the Spanish NHS.
Otherwise you would need private medical insurance and see a private doctor.

As said, lots of thing affect your access to health care.

This is a very basic (apologies, possibly ignorant) question,

If you are an Irish Citizen (i.e. with an Irish passport and therefore an EU citizen)
but have never been an Irish resident and always relied on the UK NHS (paying both the UK tax and Nat. Ins. stamp on UK income)
This description applies to many (lots of Irish nurses in the NHS for example - or people from Northern Ireland)

- how does it work then?

(Irish descent gave citizenship/passport and I am assuming, perhaps wrongly, that a right to take Spanish residence would still apply)
Really, the question also applies to that person on a less than 180 day visit (EHIC card?)
