Page 3 was for the details of the documents you were taking with you but they don't appear to want that anymore.

Just put modelo ex23 in Google and I think it directs you to the govt site. Click in that (immigration portal) and a list of all forms come up go down to ex23 and that's it

Can you still apply for residency? I thought the cut off was the 31st March.

Bit late in replying but the EX23 is not applying for residency. It is simply for changing your green registro card to the NIE. No cut off date for that.

Hi,
I've just completed the EX23 forms for myself and my wife.
I now need to fill in Modelo 790 012, I have tried but can anyone let me know how to get this form and the cost when I go to the Bank?
I have been informed it's €12 but the form I filled in had two amounts, €20+ and €9+.

Modelo 790 012
As you are already a resident, you need to tick the box marked:
Community resident registration certificate or residence card of a family member of a Union citizen
(Certificado de registro de residente comunitario o Tarjeta de residencia de familiar de un ciudadano de la Unión.)
That will show the fee as €12.00
This is if you registered with a green card prior to 1st January 2021
When you've completed the form, you will get a barcode.
You can do it with your online banking (with most banks) by simply entering the number of the barcode in the taxes section of your online banking.
Alternatively, you can do it at an ATM and hold the barcode under the laser scanner on the ATM.
As you are already a resident, you need to tick the box marked:
