Travelling to Majorca Tomorrow - Advice Needed Please!

Apologies if this question has already been answered but can anyone who has recently travelled to Majorca please let me know if we are required to complete the Spanish Health Form, travelling from mainland Spain (Rojales)? I am trying to complete the form but, where it says Country of origin, Spain is not in the drop-down bar (and this is a required field). Any help would be much appreciated. Thank you!

This link gives you the health check form (often called the locator form) for the Balearic Islands.

FCS Balearics

It's available in Castilian, Catalan and English and one of the previous port options is Comunidad Valenciana.

All the requirements are laid out on this link Travel to Balearics and you need the Travelling from Another Part of Spain option.
