Have you tried going back to the town hall and asking what has happened to your application/paperwork?
I'd say that the chances are that its been issued and is in someone else's hands.

Why did you need confirmation from your bank of payment for your private health insurance? When you paid for your PHI (you would have needed to pay for a full year up front to comply with Residencia requirements), your insurer should have given you a certificate confirming that a full year's cover was in place. We didn't have to show bank account proof of payment.

too right this is bizarre, it does all sound like a con, however you say he has been used before by others you know....so was not quite a chat in a bar.....was a recommendation?

I would A ask the others you know how to reach him and if that fails go back to the office and speak to them directly.

When I did my Residencia I had to provide a bank statement and a letter from the bank to confirm my Private Health Insurance was paid up front for the year. I received my Residencia paper on the day I went for my appointment

Elche and Alicante (plus, I assume, the other National Police stations) will supply the residence card on the day you go. The foreigners' offices (like the one at Playa Flamenca) can't do it as they have no National Police rep there so you have to wait a while to get the card sent to them.
