Changing green residencia card for TIE

I'm in the process of completing form EX23 for swapping the green residencia card for a TIE. Can anyone who has already done this recently please tell me which boxes to tick on page 2. I have had the green residencia card since April 2012 but want to change for a TIE.
4.1 Option 1."Tarjeta Initial" or Option 2."Renovación de Tarjeta
4.2 Option 2 "Residencia con certificado de registro de ciudadano de la unión" or
Option 4 "Residencia permanente con tarjeta de familiar de nacional de reino unido"
I've read conflicting information on various forums and sites so would be helpful to get advice from someone who has actually done it. Many thanks.

Hi Chrisdee, thanks, yes I've read it, but it conflicts with info on the CAB site and also another site so I'm a little confused! I just wondered if anyone had completed theirs recently and which boxes they ticked. Thanks anyway.

If this is your first TIE application then it is Tarjeta Inicial as in the guide. The Renovacion de Tarjeta is for an updated TIE which 3rd countries have to do after a couple of years (this won't apply to you).
You are exchanging a EU register certificate for the TIE so option 4.2 is RESIDENCIA CON CERTIFICADO DE REGISTRO DE CIUDADANO DE LA UNION. The other option is for family members.
