If you have completed a tax return and it has been finalised then the co-payments for prescriptions will automatically be linked to your SIP card. The pharmacist computer will charge you what you are expected to pay. Also note that age 65 doesn't matter. It is the retirement age in your home country that matters as that is the date the S1 takes effect from.

Note that other regions are different but the charges in the Valencia Region:

If you earn under 18k a year then there are no charges.
Earnings between 18k and 100k are 10% of the value of the prescriptions up to a maximum of around 18 euro (I think it's just gone up to €18.52)
If you earn over €100,000 a year then the co-payment is 60% of the costs but up to a maximum of €61.75.

So if you are a resident drawing a UK state pension and earn under €18k a year you pay nothing. (This applies to my wife as she only has the UK state pension which is under 18k a year and is over UK pensionable age)

If you earn between 18k and 100k a year then you pay 10% of the cost up to a maximum of 18.52. (This applies to me as I earn over the 18k but under the 100k a year and am also over UK pensionable age).

Last time the doctor gave me a prescription for antibiotics I had to pay 53 cents. If I was on several prescriptions coming to, let's say, 200 euro in total I would have to pay 20 euro except that payment is capped and I would only pay 18.52 for each month of prescriptions.

Of course, if I were lucky enough to be pulling in over 100k a year, I would have to pay 61.75 for those 200 euro worth of medication each month.

You are a mine of info. Does the fact I don't have Residencia yet affect the payment I have to make?

Sorry, you said you'd completed an income tax return for 2019. How can you do that if you are not a resident? It does not apply to non-resident imputed tax returns or IBI taxes, only to residents income tax returns.

Those charges and co-payments are for residents and citizens of Spain only.
There is a blanket charge of 40% if you are under pensionable age in your own country but there is no cap on the amount you have to pay. For pensioners, there is a 10% charge but, again, no cap on the amount you have to pay.

Thanks Marclff for all that. I seem to be an odd ball and may need to go back to my accountant - but thanks for information.

Jonno, reading through your posts you are doing it the wrong way round. You need to get residency first otherwise you will only be issued with a temporary SIP card which does not allow you to access the full services, just as an alternative to the EHIC.

Once you have residency, you can take that card and your S1 (take a copy as they take it off you)to the health centre in Rojales (not the one in Dona Pepa) and they will issue you a SIP card (a paper one) straight away. You can then access the Spanish NHS in full but will have to pay the co-payment charges with no cap. You also need an EHIC, a padron certificate and your passport.
In a month or two, your permanent SIP card will come through but you will still pay the co-payments with no cap until you have completed a full income tax return in Spain. A full income tax return is based on your worldwide income, not just the value of your property in Spain as the imputed tax return is.

You will get the reduced prescription payments and allowances with the cap once the income tax return has been logged by the hacienda (tax office) and the chip in your SIP card is updated.

If you have completed a full income tax return then you will be classed as a tax resident in Spain even if you haven't formally registered. That is the important thing. Make sure you are formally registered.

Thanks again for information. I have been paying tax in Spain for several years. I assumed I was just classed as a fiscal resident and paid the tax due. No doubt it will now be sorted as part of Residencia.

Hi Jonnno, Marcliff's given you a great summary, but I do hope you've already got the ball rolling on getting your TIE/residency application into the National Police authorities in Alicante before the Brexit transition deadline of 31 December 2020

It depends on your status. If you are a resident the SIP card does not expire. You will be called to renew your padron every 5 years and keeps you on the electoral roll for local elections. If you do not renew your padron then you could find your SIP card is cancelled as being on the padron is linked to the SIP card.

If you are a non-resident then the Temporary SIP card is issued for 3 or 6 months depending on circumstances. After next year it will probably not be issued as it is linked to the EHIC. For those councils who allow non-residents to be on the padron (which they shouldn't do) then it is needs to be renewed after 2 years.
