I will be going back to the uk at Christmas for the first time,I have Residencia and a sip card in Spain,the question is can I get treatment in the uk if needed?.

Yes if it's an emergency but apply for a EHIC card too

Do a search on here for EHIC and it'll tell you how to apply. Very easy.

It's not quite that black and white, it depends on how you qualify for your health care in Spain.
If it's by virtue of an S1 then yes, you can use the NHS for any treatment - not just emergencies - which becomes necessary whilst you are in the UK. You may or may not be asked to produce an EHIC. Recently it's been suggested that a copy of your S1 may be demanded too but I find that hard a bit hard to credit as it's nothing to do with the NHS at the point of delivery and in any case mere possession is in itself proof of nothing.
If it's by other means though, such as employment in Spain for instance, then you will need the Spanish equivalent of the EHIC which is called a Tarjeta Sanitaria Europea (TSE). Entitlement to NHS treatment by that route could well change if the mendacious idiot savant BoJo gets his way and UK crashes out of the EU with no deal.

Because if you're covered then why pay for insurance you don't need and which won't pay out anyway?

I appear to fall through the net for getting an EHIC card, I'm not retired so no S1 and I don't pay into the system in Spain, but I do get free healthcare from the Valencian government, but neither Spanish nor the British want to give me an EHIC, so I will just wait until I can get an S1 next July.
