Form for allowing someone to drive my car

Good morning, I believe there's a form I need to fill in to allow a family member to drive my car here in Spain. What's the form called and where do I get it from. Many thanks.

There is no official form, a hand written note on piece of paper will serve the same purpose.

Believe you also need a copy of their passport and driver's licence
Plus the N332 form is clear with all details you need

Not saying the form is not useful just pointing out that it is not in any sense an official DGT document any more than N332 are an official DGT body.
