Complaints form book for my rental Apartment, where to get?

Hello All

I have been looking around, even went to the town hall today which was suggested to me, but i can't get hold of a complaints form book for my rental apartment.

Can anyone help me please as apparently this is a legal requirement.
Hopefully will never be used though lol

I thought the local council issued these but obviously not the case.
If you Google where to buy Hojas de Reclamaciones books then several sites come up selling them including Amazon.es although they seem to be Andalusia ones from them.

Are you renting directly with the owner or via an agent?

Yeah thats the impression i got aswell hence my visit today. She spoke decent English and understood me but said no books here

I have seen the ones on Amazon but no good to me in Alicante region
