Declaration Of Tax Residence - How to fill in the form.

We are resident in the UK and our Bank Bankia has sent us forms for the above which we have never had before and I hope someone will be able to advise what should be filled in the following boxes:

Tax Code
State or Territory of Tax Residence (our house is at Montemar)
Country Code

At the bottom of the form there is:
Representative
Taxpayer Code
Full Name or Corporate name

I assume that this part should be signed by the person who pays tax on our behalf who is in Spain is that right?

Hello cant0003,

We are Non-Resident in Spain and have filled this form in for our bank at least twice now. I don't remember having to write an address for our house in Spain at all, as our State or Territory of Tax Residence for tax purposes is the UK. I do remember putting our UK address on the form somewhere.

I also think that in the part at the bottom we wrote our Fiscal representative company name. It didn't need to be signed by them though as we had to send the form back to Madrid.

I usually keep scanned-copies of these kind of things and will have a look through my paperwork over the weekend and let you know if I find a copy.

Hello Cant003,

As promised I have checked my paperwork and found the copies of the Declaration of Tax Residence, that we filled in and sent off to our bank in 2013. It states that the banks in Spain are required to obtain a Declaration of Residence for tax purposes from all non resident customers and this declaration is valid for two years. A separate form needs to be completed by each person on the account. (We sent them back in the same envelope as it was the same account, even though two envelopes were supplied).

It also states that "The address of your residence relates to the one in the UK where you are liable to pay your taxes".

I have attached a copy of the form that we filled in below (without our personal details of course) but have typed in the type of information that we supplied to help you. Also at the top of our form was instructions/guide to help the completion and I have attached that separately for clarity. I kept a copy as I knew that the next time it came to fill this in I would have forgotten how I did it.

The part where they ask for the "name and surname of your representative" I left blank, as it says in the guide "if appropriate" and, as we were in the UK, I knew that it was difficult to get a signature so in my opinion that meant it was not appropriate, anyway it was sent off and we had no come backs.

Hi, I'm in the process of completing the Bankia (caixabank) declaration of residence form and I live in the UK with a bank account in Mallorca, Spain. I see from Jan's info that the country code (codigo pais) is UK 006. Is this the general code for the UK or does is vary from bank to bank? sorry if this is a stupid question.
