If you are not a Fiscal resident IE: have filled in a tax return then you will be charged 10 % of the prescription cost

It depends on your income. Have you had a rise in payments recently?
If your income is below €18000 a year and are a pensioner then you don't pay anything. Over €18,000 a year and you pay 10% of the cost up to a maximum of €8.23 a month. Over €100,000 a year in income and still 10% but up to a maximum of €18,52 a month.

The income is linked to your tax records so they know how much to charge you. For example, anything I want will cost me 10% but my wife continues to get them for free.

Non-pensioners and income below 18,000 is no charge (these figures are if you live in the Valencia region, by the way, other regions have different rates), but those above pay 50% of the charges and over 100k pays 60% with no cap.

They could have started the co-payment again. My wife's repeat prescription was collected on the 15th of this month and she didn't pay. She did pay 10% before 2016 but that's when they went back to being free in the Valencia region.

We have been resident in Spain for 13 years and have paid 10% since the charge started. So how come people pay nothing.
We are both retired by the way.

As said above, it depends on your income. If you are resident, your SIP card will be linked to your tax returns and the farmacia charges are based on that.

Well you need to go to the Salud and get a new SIP card as the one you have will be reading that you have to pay 10% contribution
It will have TSI 002-10% on the space where your doctors name is
